Check your NHSÂ app
If your query flags up as urgent the app will tell you to call the surgery or call 111 and go to the hospital.
We will get back to you once we have reviewed your form. This is normally done within 2 working days. We will contact you to arrange a telephone or face to face appointment with a clinician.
Once your appointment has been booked it will appear on your NHSÂ app.

Expect a message from our practice team
The booking team will review your form within 48 hours and contact you to offer an appointment or direct you to the most appropriate service.
If the booking team identify that the information you have provided on your form requires urgent attention, they will advise you to call 999 or go to A&E.
If your GP Surgery is closed and you have an urgent medical problem and you’re not sure what to do you can either call NHS 111 or get help online by can typing help.111.nhs.uk into the internet. The online service is for people aged 5 and over. NHS 111 is available 24 hours a day, 365 days a year.
