New Social Prescribing Team. What is social prescribing?
Social Prescribing is instrumental in building and supporting community resilience - linking people to support and advice services around housing, finance, employment, and social welfare etc. following the impacts of Coronavirus the need is even greater.
Social Prescribing Link Workers are important and very much part of the bigger health and care system picture - supporting general practice and primary care, getting to the heart of health inequalities and harder to reach groups in our communities. Integrating with Primary, Secondary and Social Care, Local Authorities, the voluntary sector and public health.
