If you or someone you care for has a Lasting Power of Attorney (LPA) in place, it’s important to let the surgery know. This ensures we can provide the right support and care, especially when decisions about your treatment or wellbeing need to be made.

What is Power of Attorney?

A Power of Attorney is a legal document that gives someone the authority to make decisions on behalf of another person. There are two main types relevant to your healthcare:

Health and Welfare LPA
  • Covers decisions about your medical treatment, daily care, and where you live.
  • This type of LPA is only used if you are unable to make decisions yourself.
Property and Financial Affairs LPA
  • Covers decisions about managing your money and property.
  • This does not typically include decisions about healthcare unless explicitly stated.

Why Should You Notify Us?

Letting the surgery know about any Health and Welfare LPA helps us provide care that aligns with your wishes. Here's how:

Identifying the Decision-Maker

  • If you are unable to make healthcare decisions, we will know who is legally authorized to do so on your behalf.

Ensuring the Right Care

  • It allows us to work closely with your appointed attorney to make informed decisions about treatments or care plans.

Respecting Legal and Ethical Obligations

  • We are legally required to involve the person with Health and Welfare LPA when making certain decisions about your care.

What We Need

If you have an LPA, or act as an attorney for someone else, please provide:

  • A copy of the Health and Welfare LPA document (this can be provided via our secure online system or in person at the surgery).
  • Up-to-date contact details for the attorney(s).
  • Information about any recent changes to the LPA status.

How Does This Affect Your Care?

  1. Medical Decisions: If you are unable to make decisions for yourself, the attorney with Health and Welfare POA will be consulted. ‍
  2. Access to Information: The attorney may access relevant parts of your medical records to make informed decisions. ‍
  3. Ongoing Communication: It allows us to communicate with the right person without unnecessary delays.
